Benefits of Immediate Implantation

The first clear advantage is reducing two separate surgical procedures into one, which lowers the stress and discomfort associated with surgery. Placing the implant right after extraction also helps preserve the natural volume and shape of the surrounding bone and gum tissue, since bone naturally begins to resorb once a tooth socket is left empty for an extended period. Finally, this technique significantly shortens the overall treatment timeline — patients can often receive their final crown in fewer months compared to the conventional staged approach.

Who Is a Good Candidate?

Not every case qualifies for immediate implant placement. The patient needs enough healthy bone around the extraction site to achieve stable initial fixation, and the area must be free of active infection, abscess, or pus at the time of extraction. Good general health, well-controlled conditions such as diabetes, and avoiding heavy smoking all improve the chances of success.

On the other hand, this technique is not recommended for patients with an acute infection or abscess at the extraction site, significant surrounding bone loss, very thin gum tissue — especially in the aesthetic front zone — or uncontrolled chronic conditions that impair tissue healing. In these cases, we recommend delayed implant placement after the site has been properly prepared, sometimes following a bone graft.

Procedure Steps at the Clinic

The journey begins with a clinical exam and a 3D scan to accurately assess bone density and volume. The doctor then extracts the tooth as gently as possible, preserving the walls of the bony socket. Immediately after extraction, the implant site is prepared and a titanium implant is carefully placed into the socket. If any gap remains between the implant and the bone wall, a small bone graft is used to fill it and encourage new bone growth around the implant. The area is closed with fine sutures or a healing cap, and a temporary crown may be placed the same day if the implant has achieved sufficient stability.

Immediate Loading vs. Delayed Crown

It is important to distinguish between two terms that are often confused: immediate implant placement and immediate loading. Immediate placement refers to the timing of inserting the implant itself, while immediate loading refers to the timing of attaching the crown on top of it. If the implant achieves high mechanical stability at the time of placement, the doctor can fit a temporary crown the same day or within a few days, giving the patient an immediate aesthetic result. If that stability is not sufficient, it is safer to wait three to six months for the bone to fully integrate with the implant before placing the final crown — a decision made individually for each case.

Risks and Honest Success Rates

In the interest of medical transparency, immediate implant placement carries slightly higher risks compared to delayed placement, such as a somewhat lower initial stability, mild gum recession in aesthetic zones, or the need for additional bone grafting. That said, clinical studies and our own experience show that success rates for carefully selected immediate implant cases are close to those of conventional implants, typically ranging between 95% and 97% long-term when the correct protocol and follow-up care are followed.

Aftercare Instructions

After the procedure, we recommend eating soft foods for a few days, avoiding direct chewing on the implant site, and refraining from smoking, since it noticeably slows bone healing.
